If you prefer a different location, arrangements can often be made to conduct assessments anywhere within the Greater Toronto Area.

Our assessments take between 2 to 4 hours, depending on the type of assessment, and we ensure that the client knows this ahead of time. Assessments only require one visit, however, sometimes it is advisable to follow up with a re-assessment after a certain amount of time has elapsed. This helps re-enforce and ensure that all the recommendations have been applied appropriately.

Assessment fees are paid for by the auto insurance company under which the injured client is covered. This must be approved by the insurance adjuster before an assessment can be conducted. In other situations, an employer or law office will pay for an assessment.

An assessment can usually be booked for any day, even on weekends. Evening assessments are also available during weekdays. Our assessors have varying schedules that offer some flexibility so that your needs will be accommodated.

A health care practitioner or insurance adjuster must have an OCF-22 (Application for Approval of an Assessment or Examination) form completed and approved before arrangements for an assessment can be made.

An assessment can usually be scheduled within 5 to 7 days from initial contact, and a completed and thorough report can be provided within 5 business days from the day of the assessment.

Yes. Meridian Health Services has a very strict privacy policy that can be reviewed at any time upon request. There is no information as private and personal as the of health profile of an individual. Your right to privacy is a fundamental right. Every measure is taken to ensure that your right to privacy is assured.

We offer the highest quality and most accurate assessments at the most competitive prices in the market. If you are not completely satisfied with our assessment, we will work with you and/or your team until it meets your complete satisfaction.

We provide you with an invoice within 5 days of the assessment date and request payment within a 15-30 day period from the billing date.

Yes, often health care practitioners such as physicians, physiotherapists and chiropractors recommend our services to their patient’s insurance company in order to determine the needs of their client at any point during their recovery period. This allows for a more complete rehabilitation process and ultimately leads to much greater levels of satisfaction for the client, the insurer and the health care practitioner. A completed OCF-22 form must first be approved before any assessment is conducted.

Yes, the client must give us informed consent by signing a consent form before we can conduct an assessment.

Yes, all of our assessors have professional health care practitioner liability insurance.

Adjusters and Rehab Consultants aim to facilitate their clients in their return to a pre-accident functional level and often will refer a client for a specific assessment in order to hasten this entire process.

Legal representatives of clients injured in a Motor Vehicle Accident, sometimes enlist our assessment services in order to ensure that their clients receive the help that they need for their specific situation.

Employers


In work related situations, often Employers require an assessment of their employee’s functional abilities (FAE/FCE) or a Work-site Assessment to be performed in order to determine if the client is ready to return to work and if the work environment needs to be changed to accommodate the client.
